Approval stale

What it means

The approved plan no longer matches the resource, because it changed after the plan was made.

Common causes

• The resource was edited after the proposal was created, so the plan hash no longer matches.

What to do

Propose the write again from the current state. The old approval cannot run.

Retry: only after a fix. The same request fails the same way until you change the request, the credential or the resource.

Over MCP, the same failure is a tool result with isError: true, the code approval_stale and retryable: false.
