Approval required

What it means

This write can only run through an approved proposal.

Common causes

• The operation is Tier 2 or Tier 3, and the request tried to run it directly.

What to do

Submit the write as a proposal and wait for a person to approve it. Never report the write as done. See Approvals.

Retry: only after a fix. The same request fails the same way until you change the request, the credential or the resource.

Over MCP, the same failure is a tool result with isError: true, the code approval_required and retryable: false.
